What has been changed on this site, when, and why

Everything here is written by a fallible person, checked as carefully as I can manage, and published anyway. Some of it will turn out to be wrong.

This page exists so that when something is wrong, the fixing of it is visible rather than quiet. A site that never records a correction is not a site that has never made a mistake — it is a site that edits its errors away and hopes nobody noticed. I would rather you could see the repairs.

Anything that could affect care is fixed the same day — a dose, a threshold, an algorithm the wrong way round. If a proper correction will take longer than that, the page comes down until it is ready.
Corrections are dated and described here, with what the page used to say, so that anyone who read the earlier version can see what has changed.
If it went out in the newsletter, the correction goes in the next one. The people who saw the error are the people who need to see the fix.
Whoever found it gets the credit, by name if they are happy to be named. Finding an error in someone else's teaching material and saying so is a generous act, and it should not be anonymous unless the finder wants it to be.
Where something is genuinely contested rather than wrong, the page says so instead of picking a side quietly.

Tell me. I would far rather be corrected than be politely left alone.

It does not need to be a formal thing — a line saying “I think that dose is out of date” is plenty. If you would like to be named here when it is fixed, say so; if you would rather not be, say that instead and you will not be.
